Health Information Management (Medical Records)
The Health Information Management Department is located on the first floor of the hospital. Department hours
are Monday - Friday from 8:00 am – 4:30 pm.

To obtain a copy of Medical Records or have Medical Records sent to another facility
You will be asked to complete an authorization to release medical records. You can click here to print the
authorization, or you may complete one at our facility. You may be asked to show photo identification in order to
protect patient privacy and release information in accordance with federal and state laws. Please allow at least
two days to process the request.
